Many women notice that their cholesterol numbers look different in their forties and fifties than they did a decade earlier. Changes in the lipid profile — often a rise in LDL and total cholesterol, along with changes in triglycerides and in how HDL behaves — are commonly described around the menopause transition, alongside changes in where the body stores fat, in insulin sensitivity and in blood pressure.

Untangling how much of that is the transition itself and how much is age, weight change, reduced activity, sleep loss, alcohol, thyroid function or genetics is not something a single number answers. What is clearer is that these measures are worth paying attention to in midlife, because they are modifiable and because they shape cardiovascular risk over the following decades.

  • Lipid changes are commonly observed across the menopause transition, but menopause is rarely the only contributor.
  • Body composition, insulin sensitivity and blood pressure often change at the same time.

What changes

Cholesterol, body composition and blood pressure in midlife

  • LDL cholesterol. LDL and total cholesterol commonly rise around the transition. LDL is the lipid measure most consistently tied to cardiovascular risk, which is why it usually anchors the conversation.
  • Triglycerides and HDL. Triglycerides may rise, and HDL can become less protective in function even when the number looks unchanged.
  • Body composition. Fat distribution tends to shift toward the abdomen while muscle mass declines with age and inactivity — a pattern linked to lipids, blood pressure and glucose handling.
  • Insulin sensitivity and glucose. Insulin sensitivity often declines in midlife, which is part of why glucose or HbA1c belongs alongside a lipid panel rather than after it.
  • Blood pressure. Blood pressure commonly rises with age, and it is among the most important and most treatable contributors to both heart and brain outcomes.
  • Sleep, alcohol and smoking. Poor sleep, sleep apnea, alcohol and smoking all influence these measures and are worth reviewing at the same visit.

The brain is one of the most vascular organs in the body, and it depends on small vessels working well. Blood pressure, glucose, lipids, smoking and physical inactivity damage those vessels the same way they damage vessels in the heart, and vascular contributions to cognitive change in later life are well described.

Does menopause change cholesterol levels?
